Rx: Playing Orthotic for Jone's Fracture Left

Diagnosis: Left Foot Jone's Fracture / CPT 825.25

History: Young female soccer player sustained Jone's fracture while playing soccer. She has been in a walking boot for several weeks. Transitioning to soccer shoe (women's size 7) with a light weight orthotic for continued support. May resume soccer competition with orthotic in left shoe.

Plan: Fabricate X1 Blade with 5th metatarsal modification to unweight the Base of the healing fracture at 5th metatarsal. StifFlexX1 layup for unrestricted support, F1 sport profile with rocker contour to fit into a narrow soccer shoe.

Diagnosis: Fracture of 5th metatarsal mid-shaft.

History: Internet order 12/14/15 from Advanced Physical Therapy for a 26.4cm X1 Blade for a "5th met fracture"for one of their clients.

Plan: Fabricate an X1 Blade with Blade on lateral border to add support to fracture. To be used during competition.
